Radial Head Fractures

A radial head fracture is characterised by a break in the radius bone of the forearm, right up in the elbow. It most commonly occurs with a fall onto the elbow itself, or onto an outstretched hand. This causes stress to be placed on the radial head in the elbow, and when this force is traumatic or excessive, can result in a break of the bone.

It is the most common fracture around the elbow in athletes, however it does not discriminate! Radial head fractures are possible in patients of all ages and fitness levels.

Patients with a radial head fracture often experience a sudden onset of sharp, localised elbow pain. This often causes the patient to cradle or avoid using the arm. Pain can be felt at any point on the elbow as the radial head itself sits deep inside the forearm muscle bulk. Patients will experience pain and swelling, and often bruising down the forearm over the coming days. Pain will increase when weight bearing or leaning on the arm, and there will often be significant restriction in the elbow range, most notable with turning the palm to the floor or ceiling (supination and pronation movements).

Should a radial head fracture be suspected, the patient may be referred for further imaging. Most radial head fractures are stable and do not require surgery, but some displaced or compressed fractures may require a surgical opinion ASAP. The Physios at City Physio can refer you for any relevant scanning if necessary.

Radial head fractures that do not require surgery are often cleared to start physio as often as 2 weeks later! This is due to the elbow stiffening up very quickly, and it is important to preserve the range as best as possible. It is because of this reason that the elbow is rarely placed in a brace or cast in adults.
